Home and Community Based Service (HCBS) Homes

Opportunities Unlimited has 2 HCBS homes in our CARF accredited program.  This goal-driven program focuses on teaching individuals the skills they need to successfully transition to a lower level of care after receiving rehabilitation services.

Located in Sioux City Iowa, Opportunities Unlimited’s HCBS homes provide person-centered care to individuals who require support to learn skills needed to successfully transition to a lower level of care.  Trained staff provide a structured program focusing on skill building and active treatment throughout the individual’s daily routine. This individualized program allows all persons served to work on skill building in multiple areas needed to successfully transition.

  • Compensatory Strategy Development
  • Budgeting/Financial Management
  • Cooking Skills/Meal Planning
  • Appointment Setting
  • Personal Advocacy
  • Recreational and Social Skills Training
  • Community Based Functional Skills Training
  • Advocacy for Medical Care

Admission Criteria

The person to be served:

  • Is 18 years or older
  • Has a viable funding source
  • Is medically stable and does not require 24-hour nursing cares
  • Is not displaying behaviors so severe that the individual poses a threat to self or others
  • Has the potential to benefit and participate in services
  • Has a legally appointed decision maker or guardian required to make decisions and informed choices if the person is not capable of making his/her own decisions
  • Has a clearly defined discharge plan upon admission

All cases are reviewed on a case-by-case basis by the Home and Community Based Services team to identify appropriateness of the individual for this program.
