Opportunities Unlimited (hereafter: OU) continues in its endeavor to provide services in its mission-driven environment. In developing goals for the organization and in developing the strategic plan, those values of pursuing excellence, celebrating the uniqueness of all people, and demonstrating integrity in all that OU's staff do was at the forefront in the decision-making process.

 

Residential Client Demographics

  • 40 Residential clients were served in 2008
  • 48% Female and 52% Male
    According to the Brain Injury Association of America the risk of traumatic brain injury in men is twice the risk in women. Men between 15 and 25 are the group most likely to sustain a brain injury.
  • 62% - TBI, 18%- CP, 4%- Encephalitis 4%- Spinal Bifida, 8%- AVM, 2% - Stroke, 2%-Rasmussens
  • Age 16-20 9%, Age 21-34 28%, Age 35+ 36%
  • Funding for services: Medicaid 85%, VA 5%, Other 10%

 

  Residential Satisfaction Survey Results

Consumer Input Measures

In 2008, as in 2007, 72% of OU's residential clients report that in their day-to-day lives they are happy living at OU. It is worth noting that 91% of clients were happy with how the staff treated them as they work to fulfill the mission of treating the clients with dignity. The leadership team also takes into consideration that a portion of this percentage is tied into a desire to have a greater level of independence and move on.

  • 91% of Clients were happy with how the staff treated them.
  • 86 % of Families are very satisfied with the Medical services provided by OU.
  • 100% of families are satisfied with services provided by OU to their loved one.
  • 1.85 hours of non-vocational therapy and 1 hour of vocational therapy per day was averaged per client.

Disposition at discharge/transition

In 2008, 9 clients were discharged because they met their goals, 3 were discharged due to behavioral complications, and 5 were discharged into another residential home at OU.

  • 75% of discharged clients met discharge goals.
  • 17 unplanned transfers to medical facilities.

  • Opportunities Unlimited serves individuals on four HCBS Waivers; they are as follows:
    • Brain Injury 9%
    • Intellectual Disabilities (formally MR) 65%
    • Childrens Mental Health 9%
    • Ill and Handicapped 16%
    • Other 1%

  • In January 2010, 96% of Waiver clients and/or their families stated they agree/strongly agree that they were satisfied with the quality of care, provided by staff of Opportunities Unlimited.
  • In January 2010, 96% of Waiver clients and/or their families stated they agree/strongly agree they were able to receive the number of service hours needed when requested.
  • In January 2010, 96% of Waiver clients and/or their families agree/strongly agree that Waiver services meet my or my child’s social, emotional and behavioral needs.


 

Other Stake holders

  • In 2009, 93% of neighbors polled stated they have a good/excellent opinion of OU.
  • In 2009, 96% of neighbors polled stated that that OU was a good and responsible neighbor.
