Application Information
| 1. | Requests for the use of the facilities should be made and approved by the President & CEO at least one week in advance |
| 2. | In all cases, the group using the facility is responsible for providing supervision of a responsible nature, having proper insurance, and accepting responsibility for any damage to the physical facility, site and equipment which may occur. |
| 3. | The Governing Board or staff of Opportunities Unlimited (OU) shall not be responsible for any articles of clothing, equipment, or money that is left unattended. |
| 4. | Client activities will take precedence if a conflict arises. |
| 5. | ·
Facilities are to be left in as good a condition as before being used. · Floors swept or mopped as needed. · Equipment used should be replaced and cleaned if necessary. · Lights turned off and all doors checked and locked when leaving. · Any refuse, garbage, etc. should be disposed of as per instructions from maintenance. · No smoking or alcoholic beverages in the buildings or anywhere on the grounds. · No red or blue colored beverages allowed in building. · No street shoes are allowed on the gym floor unless authorized by the Governing board of the President & CEO. |
| 6. | Should use of the facilities require extra working hours, the user will pay OU according to the employee's overtime rate. OU will reimburse the employee accordingly. |
| 7. | Following is a rental fee schedule: (Wellness members receive a 20% discount on any room rental!!!!) |

| 8. | Any time there is a catered meal the rental cost will double and an additional $20 will be added for garbage pickup. |
